Hi Byron, It sounds like the writer is in a lot of pain, but that he is able to be kind to himself as he endures it. Excuse me if I have it wrong; I've never been good at pome interpretation.
Your words remind me of lines from Pink Floyd, although only because you end your words "I" and for no other reason:
